58 yo on TRT and trying to get optimally fit and healthy and increase longevity and 'healthspan.'

Medical doctor is my main gig, so I get a lot of the science of health, hormones, aging and supplements but having said that there are many mysteries of how our bodies work and I am trying hard to figure out how to optimize myself and have a lot to learn.

Regarding the term 'healthspan,' I got it from David Sinclair who wrote a great book about the state of the art of longevity supplements. He's a big believer in NAD precursor supplementation, resveratrol, and metformin.

My current supplement regimen:

testosterone cypionate 100-120 mg IM/week
nicotinamide mononucleotide 1000 mg/day
fish oil 4000 mg/day
alphaketoglutarate 300 mg/day
queceretin 500 mg/day
metformin 1000 mg/day
creatine monophosphate
resveratrol 900 mg/day
vitamin D3 2000 IU/day

Diet:
Time restricted eating, low carbohydrate

Exercise:
Mountain biking
Skiing
Swimming
Weight training

Other:
Sauna, about 5x/week

Problems:
Muscle, tendon, joint pain, intermittent
Maintaining weight loss/fat loss
Insulin resistance (elevated HgA1c)
Sleep disruption/deprivation, work related
